Find Reported Adverse Effects of a Particular Drug?

TOPIC: In this example, using three biomedical databases MEDLINE® (File 154), Embase® (File 72) and Embase® Alert (File 172), you want to find adverse effects reported for the anti-anxiety drug Paxil, especially as it relates to hair loss.

MEDLINE® is a major source of biomedical literature and Embase® contains biomedical information with a focus on drug literature. Both files provide excellent indexing — thesauri — which guide you to the right terms with a simple EXPAND command. Because MEDLINE and Embase use different indexing (NLM Medical Subject Headings MeSH® and Emtree respectively) it is best to EXPAND, browse terms and SELECT terms FROM EACH file. You can select specific adverse effects indexing from the EXPANDs or select the drug name and combine it with the two-letter codes for the adverse effects you want. The (L) connector links the heading (paroxetine) to subheadings (adverse effects) found in the descriptor field.

It is important to pick up new documents quickly. Embase Alert and "In Process" records in MEDLINE are not indexed, so in order to do a comprehensive search, a free-text version of the indexed strategy must also be part of a comprehensive search strategy.

Alopecia and hair loss are rare side effects of psychotropic drugs. There are a few case reports on hair loss associated with tricyclic antidepressants and serotonin selective reuptake inhibitors (SSRIs), but none deal specifically with paroxetine. We report on a 37-year-old female who complained of moderate hair loss during paroxetine treatment. Findings on discontinuation and rechallenge supported the assumption that the hair loss was a side effect of the paroxetine. Further investigation is needed to determine the scope of this troubling side effect.
